D-20 Registration is now closed. You can no longer register for the D-20 Certificate of Fitness Training and Exam.
The following forms must be submitted with registration. Please print and completely fill out these forms before proceeding to register for the training. Your registration will not be accepted without uploading the completed forms. Please prepare and save all of these documents and then return to this page to register for the training.
  • D-20 Applicant Affirmation Form– Download (PDF, 345KB)
    This form must be notarized. Please fill out the entire form, including both checkboxes in Section 4.
  • Employee Recommendation Form– Download (DOCX, 23KB)
    The letter must be filled out on your school letterhead using the attached template.
  • Government Issued Photo ID–
    (State Issued Driver License, Non-Driver’s License, Passport or IDNYC Municipal ID card)
  • Headshot–
    (2×2 clear color photo of your face, .jpg format (not pdf), taken in the past 6 months, no selfies, take off your eyeglasses, plain white background)
