Thank you for your interest in joining our D-20 Certificate of Fitness Training and Exam.
The following forms must be submitted with registration. Please print and completely fill out these forms before proceeding to register for the training. Your registration will not be accepted without uploading the completed forms. Please prepare and save all of these documents and then return to this page to register for the training.
EACH DOCUMENT MUST BE LABELED WITH YOUR FULL NAME AND THE NAME OF THE FILE (I.E. MOSHE COHEN, APPLICANT AFFIRMATION FORM).
  • D-20 Applicant Affirmation Form– Download (PDF, 345KB)
    This form must be notarized.

    IMPORTANT: Please fill out the entire form (2 pages), including THREE checkboxes in Section 4.

  • Employee Recommendation Form– Download (DOCX, 23KB)
    The letter must be filled out on your school letterhead using the attached template.

    IMPORTANT: You must list your job title AND responsibilities. Do not fill out your job title in the field asking what your responsibilities are. Please clearly list what you take care of. (i.e.- Job Title: principal. Responsibilities: oversees curriculum and daily operations)

  • Government Issued Photo ID–
    (State Issued Driver License or ID, Passport or IDNYC Municipal ID card)
  • Headshot–
    (2×2 clear color photo of your face, .jpg format (not pdf), taken in the past 6 months, no selfies, take off your eyeglasses, plain white background)
