Dear New York State Administrator/Principal:
The New York State Department of Health’s annual School Immunization Survey for the 2022-23 school year has been posted to the Health Commerce System (HCS) for review and submission by December 15, 2022.
Please note that only the schools with an active HCS account to access the survey will be able to submit the survey data. Schools that have not yet received an HCS account need to send an email to [email protected] or call NYS Department of Health at 518-474-1944.
