Dear NYS Principals and Administrators,

The annual school immunization survey has been posted on the Health Commerce System (HCS) on the NYS website.

While the official deadline posted is today, December 15, it is not clear whether for this year the deadline is simply tentative or a hard deadline as neither NYSDOH nor NYC DOHMH have sent any reminders to schools as they usually do. As such, while we strongly encourage schools to submit the survey as soon as they can, they should not be overly concerned if it is submitted past the deadline.

Please note that only the schools with an active HCS account to access the survey will be able to submit the survey data. Schools that have not yet received an HCS account need to send an email to [email protected] or call NYS Department of Health at 518-474-1944 for more information on how to apply for an account.
