Please select an area of practice from the dropdown menu. 

• If you have more than one practice area, please select “add another practice area” for each additional area of practice and select the additional area(s) from the dropdown menu. 

• If you have any areas of practice that are not on the dropdown menu, please select “Other” from the dropdown menu and then list that area in the box labeled “Other” that will appear.  If you have more than one area of practice that is not on the dropdown menu, please select “Add another Area of Practice” for each area, select “Other” from the dropdown menu, and list each area in its own box labeled “Other”.

• If you have selected an area of practice in error and wish to remove it before you submit your form, just set the dropdown menu for that area of practice back to “Please Select"
