Dear NYS Childcare Providers,
The New York State Department of Health (NYSDOH), Bureau of Immunization Online Day Care and Pre-K Immunization Survey deadline has been extended to May 13, 2022.
This survey is mandatory for all daycare and pre-k programs unless you already filled out the general immunization survey for your school and included your daycare or pre-k program in that survey.
The immunization survey is posted on the Health Commerce System (HCS) secure network here.
